Saturdays by appointment only.

Appointments & Cancellation
For your convenience, appointments can be scheduled by calling 602.835.2675. As a courtesy to other patients, we request office visit cancellations be called into the office at least 24 hours in advance.
What To Bring
We ask that you bring the following items to your visit:
- Driver’s license or other photo identification
- List of current medications and supplements along with dosages
- Any pertinent medical records or test results
Payment & Health Insurance
Payment is expected as you sign in for each visit. The office accepts Visa, MasterCard, Discover and American Express. We currently accept many health insurance plans. Please contact our office to verify health insurance coverage before treatment.
New Patients
You can save time by printing and completing the new patient paperwork before your first visit. Forms can be found on the Patient Portal. Call our office for user access.
